Constipation, Bloating, and Gut Comfort on a GLP-1

Constipation and bloating are some of the other most common side effects people experience on a GLP-1.

Why does this happen?

GLP-1 medications slow down how quickly food moves through not just your stomach but your entire digestive system. And if you are now eating less overall this can also include fibre and fluid both of which your gut relies on to keep things moving. Put together, and constipation and bloating become very common side effects.

Simple ways to help

Think about drinking on a schedule, don’t wait till you feel thirsty. GLP-1 medications can blunt your thirst signals, so it's easy not to drink the amount you need without realising it. Have a water bottle handy and sip steadily through the day, the goal is at least 2 litres/day. When you first start the GLP-1 keep a log of your drinking across the day so you can see how you are doing.

Add fibre gradually, not all at once. Fibre helps move things along, but adding too much too quickly, if you’re not used to it, can actually make bloating worse. Build it up slowly over a week or two including some at every meal. Include foods like:

· Vegetables (broccoli, carrots, leafy greens)

· Fruits (berries, pears, apples)

· Legumes (lentils, chickpeas, black beans)

· Whole grains (oats, quinoa, wholemeal bread/flour, grainy cereal like weetbix)

Keep moving. Gentle movement, like a short daily walk especially after a meal, helps stimulate digestion.

Try not to skip meals, even small ones. Regular, smaller meals help keep your digestive system active, even if your appetite is much lower than it used to be.

When to check in with your healthcare team

Mild constipation and bloating are common when you first start taking a GLP-1 and with each dose increase. They should improve as your body adjusts and you try the tips we have suggested. If you experience severe abdominal pain, several days without a bowel movement, or any blood in your stool then speak to your healthcare team urgently.
