Please carefully read the information on each appointment type. We offer both in clinic appointments and Telehealth (video) consults. Please see our About page if you are not sure which team member is right for you, or feel free to contact us directly.

Please indicate the issues you would like to cover during your initial appointments by adding the required information into the comments section when completing your booking.

Our face to face consultations take place in a relaxed setting. Feel free to bring a supporter with you.

WHAT TO BRING:

We will email you a digital form template for a three day food record prior to your appointment. Fill it in at the end of each day leading up to your booking and submit it, this ensures we will have the information at hand for your appointment (no need to print any forms)

Nutrition Care Dietitians are located at Gate 2/9 Thackeray Street, Hamilton Central (Inside Align Health).
